Request a Bar Card

The Utah State Bar has moved to a photo-based ID card for members of the Utah State Bar. A card is issued as part of the newly admitted attorney process.

ID Cards can be replaced for $5.00.

If you are in the Salt Lake area you can come into the Bar offices and have a picture taken and a card created for you. The walk in process takes about 10 minutes. If you are outside of the Salt Lake area, or just want to have a card mailed to you complete the brief form below and provide a JPG or PNG image that will be used as your photo.

In the case that the form is not working or you have not received and email verifying that your request has gone through,

Please send and email to onlinesupport@utahbar.org  and include the following information:

  • Your Bar Number
  • Your name how you want it to show on the Bar Card
  • A photo of you (please just a head shot)
  • Address to send it to (or if you’re going to pick it up)
  • $5.00 Payment : Cash or Check. Please send payment to:
